Is it possible to make your own "Realm" or "setting"? As in, your own continent, lore, gods, and etc. If so please leave an idea or two.

Yea. A lot of people do actually. Check the Homebrew Campaign boards for folks to aid in all your world creation needs.

Can, have and currently running it.

A great bit of advice is to bring in one other person on the project with you so you can bounce ideas back and forth as you are crafting the lore.

Since this would be your first time building a world here is my piece of advice.

If you already have players ready and want to start as soon as possible it may be best to start small with one village where the starting point of the campaign and have the players help flesh out the world as they explore.

When I started building the world my players currently enjoy I went top down making the world map first.  From there made kingdoms and their borders sprinkling a few places of interest but most of the map stays unfilled until they come across it.

As far as gods go I have always felt fine using the basic pantheon in the first campaign but my gods have a high mortality rate once the players reach epic levels so it starts to change over time.

Lore and history have always been a tough thing for me to include in the world building and other than the creation story is vague so I can change it on a whim or better include players ideas in the first campaign.

In the end though it comes down to how you do it since everyone has their own strengths and weaknesses. 

Oh and PS get a good 3 ring binder and some dividers and make sure to stay organized otherwise its easy to get bogged down and get stuck.

Just to add...

You do not need fancy computerized tools to build your world/setting.  MS Paint (yes that simple graphics program that comes with MS Windows) works surprisingly well for drawing a continent, kingdom, county, and/or village.  Or you can draw them by hand too.  And any word processing program (Notepad, Wordpad, MS Word), or pen and paper work well for anything you want to write about your world/

Yes; currently playing in one.
As many others has stated, head to the Homebrew fourm, and post your origional idea and ask for input and ideas; we have a large cummunity here and you can be assured someone will give you something you will like
