The earliest way to make yourself into something unnatural is the necropolitan template (Libris Mortis, p114). That turns you into an undead and gives you dry, withered skin and white lifeless eyes. You'd need to be at least 3rd level to survive, and you'll lose a level and some xp, but the template is +0 LA, so you'll eventually catch back up to everyone else in level.

The Mr Roboto link shows how you'd do this in a straightforward but obscene way (take Craft Construct, apply an amazing number of templates onto whatever creature you'd like (including the multiarmed template), build an Effigy of that creature, then use Magic Jar to possess it). This can be done permanently as well with True Mind Switch if you apply the right polymorph effects (Polymorph Any Object into a creature that has a mind, True Mind Switch with it, then dismiss the polymorph - originally this was used as a joke to play a sandwich, but the same idea can be used to move your mind anywhere). You can probably do something similar with grafts, symbiotes, or Savage Species rituals, but I'm less familiar with these aspects.
This sets aside why you'd ever "choose not to use items". Equipment is a large part of D&D as your levels go up.

You can call on rudimentary or instinctive knowledge of the subject creature, but not upon its acquired or learned knowledge (such as skills and feats it possesses). The same is true for the subject in your body. The mind switch brings about the following changes.
You gain the type of your assumed body.
You gain the Strength, Dexterity, and Constitution scores of your assumed body.
You gain the natural armor, natural attacks, movement, and other simple physical characteristics of your assumed body.
You gain the possessions and equipment of your assumed body.
You retain your own hit points, saving throws (possibly modified by new ability scores), class abilities, supernatural and spell-like abilities, spells and powers, and skills and feats (although skill checks use your new ability scores, and you may be temporarily unable to use feats whose requirements you do not meet in your new body).
Supernatural abilities that require a certain body part may be unavailable in your new form.
