Perhaps it's because the group decided to not enforce backup weapons and what not, because I don't recall any edition explicitly stating the penalties and advantages of fighting in cramped spaces or what not.
First edition did, but it was one of those rules nobody used because it was pointless minutia and kind of stupid.
The players I played with did. Weapon speed as well.

And in individual combat could be fun to bring back to 5th Ed (same with weapon vs. armour).

Yes, weapons shouldn't be balanced against each other. If the DM tells you that your greataxe is too large to be used in the cramped tunnel, then you need to pull out your short sword, which shouldn't ever do as much damage. And if you don't have a backup weapon then your character is an idiot.
Ha, nice, I like the way you think, what adventurer doesn't carry a dagger, or two or three.
Not, because I chose warhammer as my weapon, I can deal with all situations with my warhammy, which seems to be a popular mentality amongst a certain group.

Any sensible organism would carry a knife or two with them when doing stuff that D&D people do.
Haven't experienced the need for backup knives in the 2E/3E hybrid campaign I play in, and I actually thought that the Dragon Age PC game cinematic involving a maul being used to pierce through a creature's skull was rather brutal (much more than if the PC involved was using a blade of any sort). The 4E campaign modules I think never even take the need for tools into account, although it's something I heavily considered for my 4E Eberron campaign as well as my 13th Age campaign (wherein I made a ruling that, because one player of mine rolled a natural 1 with a maul and the fight was happening on a gangplank with another ally engaged with his target, he overswung and ended up [nearly] hitting his ally; it's not in the rules, but it certainly made sense).
So as far as I can tell, the need for backup small arms is group-specific, unless there's explicit rules stated somewhere around hand-to-hand combat in cramped spaces that you don't get penalties to attack rolls while fighting hand-to-hand or when using small weapons such as daggers and knives.
