If i hit someone with a void crystal weapon, removing it from play.
Then used void assumption, removing myself from play.
Then returned the enemy to within 3 squares of me.
Then returned myself to the world.
Does that mean the creature is perma-banned?

Not covered by RAW, expect table variation.
I'd rule no in my game - if the voidcrystalled creature saved whilst the person weilding the weapon was removed from play, the save would be treated as failed until the person was back in play, at which point so would be the monster.

Just remember that Beverly Crusher had to have outside help in getting out of the collapsing warp bubble that *she* got trapped in .....
