I've actually been trying to compose one of these myself.  This is simply for MM1, and I haven't been going back through to check some of the other ones (which I keep telling myself I need to).  I haven't even looked at MM2, but I know that some of them are out there.  Hope this is a good start.  If you want, I can email you the file as well.  Let me know.

EDIT: I think I'm going to tackle MM2 next, as I'm starting to run out of articles to search for MM1.  Plus, I already know for a fact some of the MM2 monsters I've found.
