What creature could potentially strike fear in the heart of a dragon?

A lot of it depends on the individual dragon, of course; how old it is, where it is (in lair vs out and exposed), and so forth.  And as individuals, they'll have varying levels of individual courage.

That's a pretty tall order.  Probably talking higher-orders of demon/devil/primordial sorts of things, maybe powerful undead like Liches or somesuch.
Another day, another three or four entries to my Ignore List.

Even with level parity smaller creatures don't have much of  chance to solo a dragon thanks to the 'sit on it' option. Dragons in addition tend towards arrogance. Generally the rule is find a bigger dragon.

In the old days you could get a fang dragon of equal size or even smaller due to their heavy combat skew these guys could really rip, but ever since Tiamat managed to brainwash them into thinking they needed an elemental boost the've become the much less lethal grey dragon. (note this lethality drop is in dragon vs dragon fights they still do a number on PCs not benefitting from that +5 on saves)

A sufficiently massive Behir can threaten younger wyrms.

There was also a three-headed cat-xenomorph critter in the 3e epic book that hunted dragons.

For PCs vs NPCs, perhaps, but remember that the outcome of NPC vs NPC fights are based ENTIRELY on the needs of the plot.  The DM should never sit there rolling dice for monster vs monster combat (aka 'monsterbating').

My point is while fang dragons could and did terrorize other dragons their grey descendants tend to fall more neatly into the standard dragon hierarchy of biggest wyrm in the room calls the shots.

Let's see...

Bigger Fish - As mentioned, anything bigger and/or more powerful then the dragon.  Bigger dragons, mind flayers, other far realm abominations, demons, extraplanar creatures, fey lords, etc.

PCs - Not terribly powerful on their own, but very crafty, deadly and unpredictable nonetheless.  A wise dragon would use caution when dealing with these oddities, whether they be friend or foe.  A wise dragon will also never admit to doing so.

Losing their hoard - A dragon's hoard is a symbol of its power and influence, especially in the eyes of other dragons.  And theft of its hoard is a real and constant threat, and not just from the rare band of adventurers.  Everyone from the smallest goblins to the most powerful of demon-trafficing drow aristocracy would like nothing more than to covet your Lucky Charms.

Blows to the Ego - Dragons can't abide by humiliation.  Their pride and arrogance simply won't allow it.  And even if followed by fiery vengeance, sufficient humiliation can follow them for the rest of their extremely long lives.  You don't want to be the foolish dragons that bards sing about.  Or worse, that rival dragons encourage bards to sing about.

Asking for Help - Related to the above, humility is something that dragons don't do.  And asking for help, especially from muggles, requires great humility for such a "superior" creature.  Of course, a dragon will do its darnedest to guise any request for help as a threat, a cryptic plan, or as a favor to his "allies", but to be put into a position to actually be required to say "please" to a furless ape!?  *shudders*
