Is there a weapon mod that I can get to make lets say a spear return to me after I have thrown it?

I think a Hungry Spear would be the thing you're looking for. (assuming weapon mod meant enchantment) Also: community.wizards.com/go/thread/view/758...^simple question and answer thread. pretty useful.

All enchanted light/heavy thrown weapons return when thrown.

If your weapon has the heavy or light throw property, and is magical, it automatically returns to you after you throw it.
Another day, another three or four entries to my Ignore List.

If you already have both hands full when it returns, does it just land on the ground in your square?

Ask your DM, I don't think that's covered in the rules.
Also, I don't know if its possible.

Arena fighter with battle harness and a speed drow long knife, tossed as a minor to open the encounter, then free action to draw your triple headed flail and proceed. I suppose it's semantics vs letting it return, dropping it as a free action, and drawing the flail.

I think it returns instantly, so there's no time to fill your hands first. I mean, I can Twin Strike with just one throwing weapon. Dropping it is a free action anyways, so it's kind of a non-factor.

I always imagined a twin strike with 1 throwing weapon as sort of a boomerang deal that swings out and hits 2 in the same throw

Also, I don't know if its possible.
It is, and it does - if you are unable to catch it (your hands are full, you were Stunned or Dominated as a reaction to the attack, or you simply don't want to for some reason) it falls at your feet.
