The level 11 feature of the Gladiator Champion reads:
"Whenever you bloody an enemy with an attack..."
There's no hit requirement, so which of the following count as an "attack" for this purpose?
Whirling Lunge Whirling Rend (secondary target) Whirling Step (Effect line) Slash and Slash Again

If something deals automatic damage without an attack roll, there's no attack being made.
None of sources of automatic damage you list will trigger the Halo of Destruction feature.

Of course you run into the question of whether it means attack like you said or attack meaning "attack power" which is also technically correct.

Attacking means either making an attack roll or using an Attack Power with a target line. Attack Powers are any Non-Utility Powers that damage or hinder others.
None of those Powers are damaging adidtional creature that were targeted by the original Attack and thus should not count as being attacked.
As per the RC Glossary:
RC 308 Attack: An attack roll and its effects, including any damage rolls. The word ''attack'' is sometimes used as shorthand for ''attack power''. some Attack Powers contain multple attack rolls and some Powers, such as Magic Missile, are designated as attacks yet lack attack rolls (using such Powers count as making an attack if the power has a target)

Well, now that Weapon Focus and Two-Weapon Fighting both specify "weapon attacks" I guess these don't apply to the secondary damage any more either. Man, this pretty much makes Whirling Barbarians worthless.

Far from worthless, since you can equip any 1h weapon in your offhand and not worry about its properties other than the damage die.
Weapon focus is only +3 to damage at maximum, so I don't really think it matters a whole lot, since your main target is anyway getting hit for full. Also, while raging, whirling rage adds dex to both damages, which is a big improvement IMO.
