The Primordial Maw
The head of a gigantic white ape pierces the snow and ice covered lands. It mouth remains wide open as if locked forever in rage. You feel the eyes follow you but whenever you glance in there direction they continue to stare ahead toward his long gone foe. On both sides the head five clawed fingers can be seen. They continue to twitch slowly in the cold wind.
Lore
Religion: Very Easy Check - During the battle with the gods, a primordial was slain. His death froze the northern lands. Easy Check - Artoc, the Frozen Lord, appears as an enormous white ape. Encased in the ice and snow, only his head and hands remain above the surface. His mouth is wide open as if frozen mid scream. Moderate Check - Those who have ventured to the entombed primordial say his clawed hands continue to twitch. Leading some to believe he may not be completely dead. The slow expansion of the ice wall has only enhanced people's concern. Hard Check - The Sword of Corellon severely wounded Artoc but missed his crystal heart. Removing the weapon may allow Artoc to regain control of his power and break free of the ice.
Lure
1) A cult has successfully brought Zargon into the world. In order to stop the destruction of the world they need to retrieve Sword of Corellon to slay the elder evil.
2) Archbishop Engar is concerned of the slowing growing ice wall. His northern home town has seen cooler summers and harsher winters. He wants the heroes to end Artoc permanently.
Lair
The Mouth
Once some of the party enters, Artoc's tongue attempts launches some them down his throat and proceed to block to passage. One living frost breath weapon is exhaled from Artoc's nose to deal with the party left above.
Setup: The mouth is 4x7. The outer row except for 2 squares on a 4 square side are the teeth. The 4 square side with teeth is the opening to the outside. The tongue occupies the rest of the squares. Beyond the tongue is a 2 square passage down the throat.
Teeth: Count as difficult terrain. and provides cover.
Tongue Perception DC 29: The tongue twitches a little. Trigger When Artoc has the best chance to split part of the party. Attack Immediate Reaction * Area close blast 2 one square must be a tongue square Targets: Creatures in affected squares. Attack: +28 vs Reflex Hit: Throw targets down to the stomach. Fall is 50 feet for 5d10 damage. Effect: Tongue will block the passage down the throat until DC 29 Athletics check to force it down or 80 damage is done to it.
The Stomach
The stomach is mostly a pool of acid. Several pieces of ice float in the caustic liquid.
Setup: The ice blocks are 2x2 squares of land. Place the players so that one ice block is closest to both.
Ice block: If two people stand on an ice block it drops down. Another ice block floats up somewhere else. Four element drudges rise up around the fallen ice cube.
Acid pool: At the start of a character's turn, he takes 5 acid damage if within the pool. The constant pain makes swimming as difficult as rough water.
Elemental Drudges: Swap fire resist for acid. Swap fire damage from elemental dissipation for acid.
The Heart
Getting to the heart will involve cutting through parts of Artoc. Three storm archon tempest weavers are attempting to pull Sword of Corellon from the ice heart.
Setup: One ice heart and 3 storm archon tempest weavers. An enormous sword block some of the battlefield and ends in the ice heart.
Ice Heart: Use iron golem juggernaut. Add resist 15 cold. Change all poison damage to cold. Starts immobilized (save ends).
Storm Archon Tempest Weavers: They can spend a round giving the Ice Heart an additional save against immobilization.
Sword of Corellon: Counts as difficult terrain to move through. If force moved into the sword, make an immediate saving throw or suffer 1d10 damage.
Optional Elements 1) Can Nothing Stop It?: The haunt threatens the world outside its walls (i.e., it begins expanding uncontrollably) unless the heroes accomplish a task described in the entry. 3) Guys? Where Are You?: One feature of the haunt is designed to split the party. 10) What's That Doing Here?: The haunt has one or more residents, but none are of the aberrant or shadow origin, or of the undead or shapechanger subtype.
Inspiration Note: The .hack games have dungeons where you enter through the mouth of a giant stuck in the ground. They are just standard dungeons in that game except for fleshy textured walls.

In other news, this contest has closed!
If you look past the plot and the voice acting, Metroid: Other M was an okay game. Not a great game, but an adequate one.
Not using the Metroid item collect jingle though? That, was a mistake.

Clever way to name them, but no bonus points for name dropping...or scrambling, that is. 
This is definitely a fun and somewhat challenging one to judge. Regardless of how they stack at the end - it's clear that you collectively have created a very nice pool of ideas and material for all the horror minded DMs out there (and I include myself in that category).
