Items that allow a player to damage themselves to gain a benefit should not allow the damage to be reduced, otherwise the damage is not much of a drawback due to resistance.  The sentence "This damage cannot be reduced in any way." should be added to these items. 

Also, think about changing the wording so these items cause a loss of hit points rather than damage, so that temporary hit points would not apply.

They're also interesting when combined with Hellish Rebuke, of course.

I agree on the no-prevention, but I am of the opinion that the loss of THP should be allowed.

They're still a resource that need to be generated and is expended on this drawback instead that absorbing enemies' damages.
