Polymorph powers change a creature’s physical form in some way. For instance, a polymorph power might transform a creature into a mouse. Unless otherwise noted, a creature uses the following rules when it is affected by a polymorph power.
One Polymorph at a Time: If a creature is affected by more than one polymorph power, only the most recent one has any effect. The other powers’ effects remain on the creature and their durations expire as normal, but those effects don’t apply. However, when the most recent effect ends, the next most recent one that is still active applies to the creature. Example: If a druid uses a polymorph power on herself and then a monster uses a polymorph power on her, the polymorph effect of the druid’s power is suppressed until the monster’s polymorph effect ends on her.
Changing Size: If a polymorph power reduces a creature’s space, the creature does not provoke opportunity attacks for leaving squares as it shrinks. If a polymorph effect would make a creature too large to fit in the available space, the effect fails against the creature, but it is stunned (save ends). Example: If Keira is crawling through a narrow tunnel and a polymorph effect tries to make her too large for the tunnel, the effect fails, but she is stunned until she saves against that effect.
Death Ends: Polymorph effects end immediately on a creature that dies.Spoiler:

Quick question about Hexblades and the character builder. For some reason, the CB lists both the pact weapon and implement on the power cards. I was under the impression that all your melee attacks were performed using your pact weapon. Can you use your implement instead?

Instead? No.
You use both. Hexblades are different from other classes in this regard, you use both your implement and your pact weapon to make the hexblade attacks. It's why they have both the Implement and Weapon keywords.

That makes no sense. How does that play out mechanically? I mean, the CB lists both of them independantly. Without any feats or anything, sure they're identicle. But once you start adding in expertise and focus feats, they start becoming uneven. For instance, someone in my party is using a Fey Pact hexblade in my current game. On any attack that he can use his Hexblade for (With the exception of his MBA) it also lists his stats when using the implement, and his attack bonus is actually higher with the implement.

Here's the relevant rules text from the Hexblade class (emphasis added):
PACT WEAPON While you hold your implement in one hand, you can use a minor action to manifest your pact weapon in the other hand. The pact weapon you create depends on your pact. Your pact weapon persists until you no longer hold either it or the implement, or until you dismiss it as a free action. You can make weapon attacks with your pact weapon, using its proficiency bonus and the appropriate damage die. Your pact weapon shares your implement’s enhancement bonus, critical hit effect, properties, and powers. The weapon cannot be enchanted. When you use a power associated with your pact weapon and the power has both the weapon and the implement keyword, you are considered to be wielding both your pact weapon and your implement for the purpose of feats and other game elements.
I haven't used the CB for Hexblades so I can't verify whether its doing any of the math right, and it might be worth double checking.
For any weapon attack, such as a melee basic, you can use the Pact Weapon and its proficency bonus and [W], but use the enhancement bonus, critical hit effect, properties and powers of your implement. However if its only a weapon attack, any feats that apply to the implement (implement expertise, for example) would not apply. Weapon expertise would apply. For warlock powers that are associated with the pact weapon (having it manifested is a prerequisite) and have both the weapon and implement keywords, any feats or game elements that refer to your weapon or implement can apply. So you could apply either weapon expertise or implement expertise to that attack, if you have either feat. You can't apply both as they are both feat bonuses and do not stack. But you could apply Implement expertise and Weapon Focus to that attack and both would apply, as it is both an Implement attack and a Weapon attack and they are bonuses to different rolls.
This also effects other game elements, such as triggering a Black Pudding's Split power, because it counts as a weapon attack, etc.

It has some significant issues. It gets it right if you're using a non-weapon implement and your pact weapon and have the implement equipped and selected for 'show on power cards'. It uses the pact weapon and the implement for calculation for feats and such, and does everything properly.
It's completely wrong if you use an implement that's also a weapon (like a dagger or staff), and it will take that item's [W] and proficiency and all other properties and ignore the pact weapon completely.

The Githyanki Mindslicer 'Psychic Barrage' power has the following hit line - 1d6+3 psychic damage, and ongoing 5 psychic damage (save ends), and the target can’t use daily or encounter powers (save ends).
I understand that this prevents the use of daily/encounter powers from class and race - does it also prevent the use of daily or encounter powers from magic items?

ANY power which has the usage 'daily' or 'encounter' cannot be used. Any power, regardless of source.
