If you have cover relative to any enemy and move from one of it's threatened squares do you provoke an Opportunty Attack ?
(YOu didn't in 3.x but this is 4.0)

Yes. The OA block on PHB 290 says "Able to Attack: You can't make an opportunity attack unless you are able to make a melee basic attack and you can see your enemy". If you check out the Cover definition on PHB 280, it explains how to determine cover. Even Superior Cover allows attacks (albeit at -5).
Now, if the player has total concealment from the enemy (invisible, stealthed, etc), then no, there is no OA.
