Is it possible to make melee attacks from another origin square or teammate? Just like you can make arcane ranged/blast attacks from your familiar.

You can't make Arcane Ranged/Blast attacks from your familiar unless you have a feat/feature/power that specifically says you can, such as the Familiar Bloodsmith Paragon Path.
There are a number of Artificer (and I believe Shaman and Warlock?) powers that allow you to use an ally as the origin of your attacks. They're pretty much all terrible since they take up a daily power slot.

Well, some of the familiars have the ability to "once per encounter, determine range etc. from the familiar's square" - Arcane Eye, for instance. Something like that, but for melee, would be enough - i'm trying to make a pseudo-lazy Bard/Warlord/War Chanter and just don't want to whack an enemy myself with a Victorious Smite etc. Actually, just found one - Greater Dancing Weapon at-will, but waiting till 15th level sucks

Would having both ferocius frenzy and vicious ferocity allow you to make an at will attack against each adjacent enemy with a -2 attack penalty? Or are they mutually exclusive? Spoiler:

Vicious Ferocity
Paragon Tier Prerequisite: 11th level, minotaur, Ferocity racial trait Benefit: Whenever you would make the melee basic attack granted by your Ferocity, you can substitute any at-will attack you know for the basic attack.

I'm no rules guru, but that first one says "instead of making the attack Ferocity grants you" while the second says "Whenever you would make the melee basic attack granted by your Ferocity" so if you do the first you can't do the second, and if you do the second you cant do the first.
And a question: What sort of things do and dont apply with the second damage roll of Hellish Rebuke?

The lack of "per attack" on the extra damage leads me to believe no, but wording is a tad ambigious.

The counterargument is that the same Hit: line is used for all the attacks. And it doesn't change...

By that same counter argument the first sentance could also apply to each attack causing quadratic appliaction of the attack damage  Edit: this and a multitude of other powers ( such as twin strike) gives clear Precedent that the hit entry can be read as one singular hit entry for powers with multiple attack rolls for the same attack.

STARBLADE FLURRY Prerequisite: 11 th level, monk. flurry of Blows class feature Benefit: When you use your flurry of Blows power. you can add a single target within 5 squares of you by throwing a dagger or a ,shuriken that you have In hand. Doing so doesnt, promote opportunity attacks.
Does that mean I lose the dagger when i throw it? Or does it magicaly come back to me?

It depends. All enchanted weapons (+1 dagger works just fine) returns. Non-magical dagger... wait, why do you use nonmagical dagger at 11+ anyway?
