It only needs to be on your person. Reason dictates that it should be reachable and you should be able to put it in your mouth to use it, but it is not strictly required by the rules.

Mebbe you rig up one of those Bob Dylan-style hands-free harmonica holders?

That would constitute it being within reach and being able to put it in your mouth. :D

If you Crit, and then use the Theme power "Essence of Power" from the Student of Evard theme, is the Damage from Maxed from it? Or do you have to roll it?

I only see an ability from the Student of Evard theme called "Essence of Death". Since the damage it deals is extra damage, the damage is dealt as part of the damage of the attack, and all of the attack's dice are maximised. Only effects that come solely from the crit are rolled, and Essence of Death is not solely from a crit.

Short answer: yes, it is maximised. Bonus: the d6 you take still isn't. Cool.

You would only roll damage that comes forth from the effect of a critical hit actually taking place (Magic item critical hit dice, etc).

Hybrid Warlock (Eldritch Strike) or Hybrid executioner will do the trick

How does the Executioner work here? The at-wills don't seem to give me a special: mba.

INT/CHA race with Hyrbid Bard|Swordmage and take Intelligent Blademaster.  Now, your basic attacks use your INT for attack and damage.

Hybrid Executioner can use Dex for MBAs. So you can stack up Dex/Cha and use your MBA, plus Executioners get an extra d8 on one-handed MBA attacks.

Do I add the enhancement bonus of Symbol of Divine Reach to all attacks, or only to divine attacks?

Only attack powers with the Divine keyword are Prayers, so it only works for those.

So, let's say I get howl of fury, a Barbarian primal power area of effect attack, and then got Hammer rhythm which allows me to do damage with a Hammer even if I miss equal to my con mod, and then used Howl of fury, which does thunder damage against minions and my attack missed, would I automatically hit all the minions, conceivably killing them?

I don't know if it counts as a weapon damage attack, so therefore including your hammer, but the basic attack starts off with 1[W] and you have to include the target in the blast, so I'm unsure if hammer rhythm would apply in this situation to the targets of the blast.

When you miss with an attack, everything not on an Effect: line is completely expunged from what the power does.  All Hammer Rhythm does is give you a Constitution modifier damage poke as a consolation prize.
