This is my effort to create a compiled, printable list of all rituals appearing in official sources. It will forever by a work-in-progress, of sorts, but I will not post any updates that I feel are incomplete. (This thread mirrors this project's home-thread at ENWorld.org. It appears here for additional exposure and feedback.)

Offering suggestions about how I could make this index more helpful to you or others;
Indicating any places in the official text of a ritual where you believe the rules aren't clear and need to be updated / errata'd, and the manner in which you believe the text should be updated. There is doubtlessly errors in this list, even though I've tried to be very thorough. I intend to include all rituals appearing in official D&D publications, including all books and DDI materials. I plan to update this list to match official updates/errata. I welcome any suggestions, corrections, and especially omissions! (Feel free to post them in this thread.)

Added all rituals appearing in "Chapter 5: Rituals" in the Forgotten Realms Player's Guide, as well as updating the three rituals that had been previously posted from an online excerpt of the book;
Added the ritual single that appears in Adventurer's Vault;
Balanced the layout so that each page is mostly full, and so that the split between pages does not divide any group of rituals of the same level.

A month and a half after I began this project, I've finally gone to the trouble of mirroring it here at Gleemax. I've done so because it's a labour of love, for my own game and for the community, and so I want more people to see it. I think it would be dandy to get more feedback, too; I consider this project very open to suggestions and constructive criticism, and I heartily encourage everyone to leave some!
At the moment I'm looking for feedback particularly regarding switching to a landscape-layout for my next update. Some folks have been asking me to add columns for Market Price and Component Cost, and switching to landscape would give me the space to do that without cluttering the page or decreasing the font size. I'd also like to add a very brief description of each ritual's effects, if space permits.
A player needs to read the sections for building his or her character -- race, class, powers, feats, equipment, etc. But those are PC-specific. The above list is for everyone, regardless of the race or class or build or concept they are playing.
Sorry about that, I didn't even realize an ENWorld account was necessary. I'll attach a separate copy here.

DMG has numerous Rituals mentioned, that aren't finished. Most are in the Templates section. Sphere of Annihilation is in the traps section. Also un-finished.
MM has numerous Rituals mentioned, that aren't finished
MM Lich and DMG Lich sections, when combed though, yeild: Unnamed Ritual to make a Lich's Phylactery, probably level 14, for Lichs that don't have the Phylactery yet. Unnamed Ritual to make a Lich without a Phylactery, might be level 14, might be lower; see 1st line of the MM entry. Clearly making ~lesser Lichs, is almost entirely DM Fiat, never-the-less, a Ritual of some sort is specifically mentioned.

Not so long ago, I had a conversation with another fellow regarding the very things you are mentioning. Unfortunately, (or perhaps fortunately, for me at least), I ultimately decided not to include any "undeveloped" rituals in the project. You see, this fellow had suggested that I should, and showed me a list of all the undeveloped rituals mentioned in the Monster Manual. There must've been fifty or more entries from that one book! Moreover, none of these "rituals" really had any information about them, not even names. I know there's been about a half-dozen appearing in H1-H3, numerous ones in FRCG, and probably countless more in the eight-hundred-ought pages of Dragon and Dungeon so far released, not to mention the other books out already.
My decision not to include these rituals is a matter of pragmatism, really. First and most importantly, the primary objective of this project is to compile all the officially developed rituals together, for ease of selection for those with the Ritual Caster feat. It wouldn't make sense to include (functional) non-options on a list of options, especially given that these non-options likely outnumber actual rituals by four- or five-to-one. Secondly, attempting to index these undeveloped rituals poses a unique problem: How does one go about indexing alphabetically, by level, entries that have no specified name or level? In the interests of maintaining this project as one that indexes officially developed rituals, I can't personally develop rituals for no other purpose than to fill-out entires in a database; that would be unofficial, potentially unbalancing, absolutely daunting, and substantially a wasted effort. I hope, then, that you understand why I've declined to handle this matter within the scope of this project.
I'm not really sure why some rituals have been developed already and others haven't. For example, there are only two (developed) rituals that appear in the FRCG, and IMO, both are so specific that they will hardly ever see use in games. (YMMV, of course.) In contrast, the ritual to create a Lich's phylactery (if not Enchant Item) would probably see widespread use, even if not often. That said, I definitely expect many of the undeveloped rituals hinted at will be developed or elaborated upon in future products, especially monster splatbooks, given how many of the undeveloped rituals hinted at so far are related to the creation of various creatures. (Open Grave, available in early 2009, is an undead-themed book where I expect to find some new undead-creation rituals.)

FWIW, if you're into online D&D forums, (and you're here, so you must be), you should check out ENWorld as well. If you've never heard of it before, it's a major pillar of the online D&D community, and a lot of folk from here post there as well, including numerous WotC Staffers. Suffice to say, it's a highly-respected D&D forum, and you ought to check it out! (It's the #1 place for D&D news, as well.)
But you're not here to be sold on joining another forum, you're here for a PDF Ritual Index. Any ideas about where else to host it, so that you won't have to register to download it? It needs to be somewhere permanent.
