Loka: the Customer Help Center provides tons of helpful information, incl. an answer for your case; see here for details. PS: I'll move his thread to the DCI board, as it's not a DCIR-related question.

I, unfortunately, forgot the password for my DCI number and the email address currently associated with it number hasn't existed since 2001. I really want to get on and check my ratings, but I can't seem to find any form on the DCI site that lets me change it. Who do I need to contact or where do I need to go to rectify this? Can one of the guys at my local game store do it?

You can request Wizards Customer Support for a new password to be sent to your new email address. You must provide as much vital information as you can, including your [LIST=1] DCI Number Name Birthday New EMail Address Present Mailing Address and any information of your previously attended DCI sanctioned events.
You must email them using the webpage, the topic category should be DCI subtopic Password Requesta

I recall there being a way to look up a player's dci/rpga number for them using their full name and location.
Is this still possible?
If not, is there some other way to quickly find a player's number for reporting?

Lord_Darquain: you can use the DCIR for that. Open a tournament, choose "Players" under the "Edit" menu, then click the "DCI DB Lookup" button to pull up the search screen. If the player is not in the downloaded database (current version # is 37), the player can get his/her DCI# from the Customer Help Center; see here for details.

Hmm, I mostly run RPGA events, which can't be reported using the software (as far as I know). Can they?
Thanks anyway.

I downloaded it, eventually set everything up, and found their numbers for them, thanks again!
