If there's interest, I'd be willing to sticky this thread, or if you wanted to split this into the "actual" compendium and keep this as a related "discussion" thread I could sticky the thread, and mark this discussion thread as do-not-archive (so both stay "live" indefinately).

All classes: Drawing and sheathing weapons is a minor action, it can't be done as part of a move action as per 3.0/3.5 and a duel wielder can not draw both weapons with the same action as per 3.0/3.5.

Ranger/Fighter: Twin strike, dual strike, and sure strike (as well probably a few other powers here and there) do not add your attribute to damage.

GMs: Hard corners do not give cover if both targets are at the corner, all LOS and cover determinations work the same now, rather than melee and ranged being checked differently like they were in 3.0/3.5.

LoS in general is very simple and yet I still see so many people get it wrong. During a game this weekend a GM confused his players soo much on it they had to start randomly asking if they could see their target yet, and I got into an argument into how far past a hard corner I could see (the answer is infinite, but the GM had it in his head that you could see a few squares out, but not farther).

Shift/walk past a hard corner (this may be the biggest 3e holdover rule that everyone seems to get wrong).

I thought in neither 3E or 4E could you move diagonally across a hard corner, am I missing something?

Barbarian: Swift Charge is still a charge, so it ends your turn barring the use of an Action Point.

You do not get the 1/2 level bonus to damage rolls.

Weapon Focus feat adds +1 to damage, not +1 to attacks. Also it does not stack with Dwarven Weapon Training or Eladrin Soldier.

You must make a Stealth check to become Hidden, even from blinded enemies or while you're invisible.

You must be able to see your target to gain combat advantage, and to make an opportunity attack against it.

Forced movement into any kind of hazardous terrain (not just pits or precipices) grants the target an immediate save to fall prone instead of entering it. (This info is in the DMG, so a lot of players miss it.)

Action points can be spent any time during your turn, and they do not automatically end your turn.

There's arguments for both sides. Which means it doesn't belong in this thread.

I agree that it does not belong here, but I'm still interested in seeing an argument for your side. Can you post one in the thread I mentioned (here)?

Diagonal steps count as a single square, not only for calculations of movement, but for all distance calculations, including determining relative closeness of targets, directness and end-points of charges, and eligible directions for Push and Pull forced movement.

This is intended to be a list of things people have seen characters do that are flat out wrong without any gray areas. Feel free to post additional items and I'll edit the following posts appropriately.

DMs asking for rolled Perceptions and the like when Passive scores should be being used (exacerbated by the modules that also make this mistake).

Looks like now we can add: Spellscarred use implements they are already proficient with for their implement powers.

(That's just some friendly ribbing of MwaO :D)
Sorry WOTC, you lost me with Essentials.  So where I used to buy every book that came out, now I will be very choosy about what I buy.  Can we just get back to real 4e?

Check out the 4e Conversion Wiki.
