What happens if a creature has multiple instances of Madcap Skills on a single creature? If it has two on it it obviously gets +6/+0, but what's the minimum number of blockers required? I seems like it should be 3 or 4

Only two blocking creatures are required. Madcap Skills #1 asks "are two or more creatures blocking?" and gets a "yes". Madcap Skills #2 asks the same question and also gets a "yes". Therefore, all the blocking restrictions are being obeyed.

If one effect says it can't be blocked except by two or more creatures and another effect also says it can't be blocked except by two or more creatures, that just means it can't be blocked except by two or more creatures.

The creature will have multiple effects requiring it to be blocked by at least two creatures. Most of the time, this won't matter -- I can't think of a situation where it would. If Madcap Skills used the phrase "additional creature" instead of being explicitly two, then you'd have a different end result.

Still 2. Both auras set independent blocking restrictions, which are both fulfilled if the defending player blocks with 2 creatures.
I'm a Rules Advisor now  "Simple questions" usually need rather complex answers, while complex questions often come down to no more than a simple "yes" or "no". Some of my favorite Flavor texts:

can't be blocked by only one creature
having this restriction more than once has no additional effect
