When playing with kaalia of the vast how would exalted work?
Would she get the bonus from Exalted?  She is declared as a single attacker before her ability kicks in?

If she is the only creature declared as an attacker, then exalted will trigger. Creatures put onto the battlefield attacking by her ability will not interfere.

Yes, exalted will trigger since you only declared her as attacking.   The token doesn't interfere with that.

the creatures put onto the battlefield by Kaalia are attacking but they never attacked
besides, Exalted triggered before they were on the battlefield, so they can't interfere with it anyway

Therefore, if the creature you put on the battlefield attacking is Sublime Archangel , her Exalted ability (and the Exalted instances she gives to other creatures) will not trigger.
