So what happens when someone enchanted with Curse of Echoes  cast a spell while Hive Mind  and Eye of the Storm  are on the battlefield?

Unfortunately, VCLs do not currently have the tools necessary to take moderation actions directly. VCLs submit their actions to ORCs, who then actually perform the action. This processing can take between a few minutes and several hours, depending on how busy/attentive the ORCs are. If you see something that needs VCL attention, please use this thread (MTGO) or this thread (paper) to make a request and a VCL will look at it as soon as possible. CoC violations should be reported to Customer Service using the "report post" button. The ability triggers when a player casts an instant or sorcery *card*. A copy of a card (such as those generated by Eye of the Storm itself) won't trigger this ability. The card that was cast is exiled and won't resolve, but the player will get a copy of the spell from Eye of the Storm.
This would include all copied spells so the players getting copies will not trigger eye of the storm.

Copies made by Eye of the Storm are "cast without paying their mana cost", so they would trigger the other two.

Here's what might happen: You are enchanted by Curse of Echoes and your opponent controls Hive Mind and Eye of the Storm , which has exiled Lightning Bolt  , Giant Growth  , and Ancestral Recall  .
- You cast Dark Ritual
 .
- All three blue things trigger and go on the stack. Your opponent chooses the order.
- Hive Mind has your opponent copying Dark Ritual . It's not cast, so nothing triggers.
- Your opponent's Dark Ritual resolves, giving him
 .
- Curse of Echoes does the same thing that Hive Mind did. This guy's now got six mana.
- Eye of the Storm's trigger resolves. Dark Ritual is exiled. You can cast any, all, or none of Dark Ritual , Lightning Bolt , Giant Growth , and Ancestral Recall . In whatever order you like. Any one you cast will each trigger the curse and the hive (since they are cast), but none will retrigger the storm (since they are not cards).
- Once your opponent is done resolving his spells (and, ostensibly, laughing maniacally), you can finally handle yours.
As far as I can tell, the Curse and the Hive are fully capable of making a copy of the spell you cast, even if Storm exiles it first. They'll just use last known information.
