If a creature has exalted already on it and another card comes out that gives all other creatures you control exalted does that mean the first creature has double exalted and each creature that comes into play on your side with exalted gets double exalted?

Yes. As a triggered ability, multiple instances of Exalted will all trigger when appropriate.
Another day, another three or four entries to my Ignore List.

Yes, those creatures will have two instances of exalted. If a creature attacks alone, each instance of exalted will trigger.

In essence you are correct. The exalted ability does stack, so if a permanent has exalted, and another permanent gives that first permanent exalted then the first permanent will now have 2 instances of exalted. Both instances of exalted will trigger if a creature attacks alone and each will apply its bonus seperately.

Yes, check out the wording on sublime archangel : Exalted (Whenever a creature you control attacks alone, that creature gets +1/+1 until end of turn.) Other creatures you control have exalted. (If a creature has multiple instances of exalted, each triggers separately.)

Let me see if I get this...
I have 5 creatures out. 1 without exalted 3 has exalted once 1 has exalted twice
If I attack with only the first creature, it would get +5/+5. Correct?

Five instances of Exalted go on the stack. Each will resolve to give +1/+1. It will eventually get +5/+5.

And it doesn't matter which creature attacked, as long as only one attacks it would get the +5/+5 from all exalted triggers.

More proof that they can have more than one instance of an ability. Maelstrom Wanderer has Cascade twice Cavalry Master gives creatures with Flanking another instance of Flanking
