You find some way that both players can agree on as a sufficiently random method, and then use that to determine the card that gets discarded.
Most commonly, your opponent holds up their hand, and you pick a card without looking at it. Or, if you'd prefer, you can assign each card a number on a 6-sided die, and roll to see which card is discarded.
It doesn't matter how the random discard is handled, as long as it's actually, y'know -- random.
Edit - In response to below, that reminds me. Yet another method would simply be for a player to shuffle their hand, and then place it face-down and have their opponent point to a card. Random, and avoids the problem of having a (bad) poker face! :D

Indeed. If you're going to do the pick-a-card method, the player should shuffle their hand and fan the cards face-down, so that neither player knows their order when the card is picked.

Another way to pick a card at random is to throw them in the air, shoot at them with a shotgun and discard the card with the most holes.

The player shuffles their hand and then, without having looked at the cards, holds them face down on the table while the other player chooses one. No dice rolls, and no need to deal with "poker face".
