If opponent attacks me with his 12/12 creature and i declare
Phyrexian Obliterator
as my blocker how many permanents must opponent sacrifice?
One because there is one source, or 12 because source dealt 12 damage to obliterator?

The number of sources does not matter, only the amount of damage dealt to the Obliterator. 12 damage dealt means 12 permanents to sacrifice.

"Simple questions" usually need rather complex answers, while complex questions often come down to no more than a simple "yes" or "no".
