The ninjutsu ability doesn't work from the Command Zone. Ink-Eyes needs to be in your hand to use it.
Any general costs as normal when cast from hand (it might be there because of evacuation  or something simular). There is no general tax when casting from hand.
Hunh, well, that's sort of a problem. How do I get Ink-Eyes into my hand so I can use her properly, would be my next question.

how many relentless rats can you have in EDH/Commander i really need to know... i've talk to many people and they say as many but some say one... so how many?

When Kaervek the Merciless  is in play and someone casts their commander, Captain Sisay  for the 3rd time... is the CMC still 4 or does it go to 8? Thanks

The converted mana cost is still the same. The extra cost is a like a tarriff to be brought back in.
