If your opponent takes control of a creature with equipment attached, does the equipment go with it? Also, if it does and that creature dies, does the piece of equipment stay with the opponent or does it go back to the owner?

It stays attached, but does not change controllers. The equipment's controller can still move it back to his own creature by paying the equip cost.

When the opponent gains control of the Creature, the Equipment will remain attached to it.
However, the control of the Equipment has not changed; The Equipment's controller can play the Equip ability to move it to a Creature they control.
