You know, that flimsy piece of plastic they tear off the form you fill out when you first sign up with the DCI? Yeah, I lost it.

BUT I have this nice little plastic card that says "Magic: The Gathering Player Rewards" and a picture of Sarkhan Vol that I got in the mail. On the back there is a bunch of text starting with "The person whose name appears on the reverse side hereof is a member of the DCI and agrees to comply with all DCI rules..... bla bla bla", the Wizards of the Coast logo, the DCI logo, my name, a 10 digit number, and another 5 digit number over a barcode.

Does this count as a DCI card? Like, can I sign up for FNM with it? I'm just getting back into Magic, so I haven't tried yet.

Yes you can. You actually just need your DCI Number to sign to FNM or any tourney sanctioned by the DCI.

I don't remember my DCI number, though. Is it the ten digit number on the back of the Player Rewards card?

Show up to a sanctioned event, and just tell them your name. Whoever is organizing the event can look up your number using the DCI Reporter software.
