Would it be possible to make a Phantasmal Image into a legendary creaure, creating a possibility of have four legendary creature out at once? or Clone and Cryptoplasm

Among other characteristics, Phantasmal Image copies the original's name, which is what the Legend Rule keys off of. Sakashima the Impostor and Lazav, Dimir Mastermind keep their own names, so they would do what you want.

"Proc" stands for "Programmed Random OCcurance". It does not even vaguely apply to anything Magic cards do. Don't use it.

No.  If there are two legendary creatures on the battlefield with the same name, including ones which are clones, both of them are sent to their owner's graveyards.

When the magic 2014 rules go into effect, there will be a modification to these rules. Under the new rules, if a single player has two or more legendary creatures with the same name, all but one is sent to the graveyard. You still won't be able to have 4 of them like you wanted, but you can clone your opponent's legendary creature and have one copy for yourself.

So it is possible with a black and blue deck to have 3 of the same legendary creature but with different names
Actually, it's possible for any color deck to have as many identical Legendary creatures on the 'field as it wants: behold!

But yes, with some shenanigans, you can use Sakashima and Lazav to duplicate a Legendary creature twice.

Also, if it matters to you, it still comes into play for the purpose of triggered abilities. However, it dies immediately, and you can't use activated abilities to respond to its death. However, you can use activated abilities to respond to the triggered ability.
