"Equip" means "Pay cost: attach this to target creature you control. Do this as a sorcery."

You target the creature, it's sacrificed.

How to autocard :
Type [c]Black Lotus[/c] to get Black Lotus.
Type [c=Black Lotus]The Overpowered One[/c] to get The Overpowered One.

if you try to move it from another creature it stays attached to the original creature

the Illusion is sacrificed before the Equip ability resolves

Are there cards that can counter Illusions' sacrifice ability?
Those work, but are there any permanents that can counter abilities?
Activated abilities, yes. Triggered abilities, no. Unless you count Stifle or Trickbind on a stick.
If you can get that equipment into play with something like Quest for the Holy Relic or Stonehewer Giant, you could attach it to your Phantasmal Dragon without ever having targetted it.
something like Humility or Sudden Spoiling would remove the sacrifice if targeted trigger
