Bloodclaw weapon and multiattack powers

How does the Bloodclaw weapon combine with powers that grant multiattacks?
I see three possible scenarios.

1. Take damage once and apply damage bonus to all attacks.

2. Take damage per attack and apply damage to each attack. This however, would imply taking an action (or multiple actions) within an action.

3. Take damage once, and apply damage bonus to the first attack.

Could anyone clarify this for me please. 
4. Take damage once, and increase the hitting attack's damage you decide to apply the Bloodclaw power to.

The Bloodclaw's effect is an encounter power you use when you hit with an attack and affect the damage of that hit and is then expanded.

AV gives it as an At Will free action 
Yes the Bloodclaw Weapon got a Rules Update:

Bloodclaw Weapon
Page 65: In the item’s power, replace “at-will” with
“encounter,” replace “before making a melee attack
on your turn” with “when you hit with this weapon,”
and delete “If you hit” in the last sentence. This
change brings this item in line with damage output
expectations.
