Combat Situations Outside of Combat

I've noticed that occasionally combat effects overlap and run into non-combat situations. In the books, it says that combat effects usually last about 5 minutes and that as long as the characters don't take a rest between encounters that they continue to reap the benefits or penalties for the full 5 minutes. But it is unclear how to deal with effects that deal with saves outside of the combat environment.

In death saves it says that if 3 failures are accumulated before a rest (short or extended) then the character dies. I am assuming this means that if a character can rest before the third fail, he does not have to make that save any more. It doesn't, however, state whether or not that character has to actually heal in order to avoid making any further death saves.

Under normal saves, it doesn't say anything about making further saves after the encounter has ended or in non-combat situations. Does the save effect end with the encounter? Does the character continue to make saves? If so, at what rate? According to the material a turn lasts about 6 seconds... Should the character continue at that rate, just rolling saves constantly until successful? If that's the case, one would assume that eventually the player will succeed and that the effect would virtually just end with encounter.

Thankfully this situation doesn't crop up very often, but it does rear its head every now and then and I'd like some opinions and suggestions on how to effectively deal with it when it does.

The rest doesn't start until everyone is safe.  If you are making DSTs, you are not safe.  The encounter continues, in rounds, until everyone is at least stable, if not above 0hp, and not taking any form of ongoing damage.

Don't forget that the other characters can use heal checks to grant Second Wind (DC10) or Stabilise (DC15) unconscious characters, or can feed them potions of healing or whatever as a standard action.

With the exception of ongoing damage this is pretty much accurate. Ongoing damage, however, is worth making the saves for. There's a chance they will take enough damage to matter for healing surge use, or future encounters.
Or if people are under a petrification threat, for example.  There's some effects that are slowed, first failed save immobilized, second failed save petrified.  If you just call the encounter over when the person is only immobilized, you've removed the threat for no reason.

Continue the encounter until the threat is resolved.  The threat doesn't necessarily mean the enemy monsters.
