Why can't Phyrexia enter Dominaria? And how can it planeswalk?

So I've read everything up to Prophecy incuding The Thran and am a bit confused about the abilities of Phyrexians.

1. I understand one needs to be a planeswalker or have assistance of one to  planeswalk. This is why Yawgmoth was trapped in his Phyrexia for those thousands of years. But then, in Planeswalker and Bloodlines, Phyrexians freely travel between planes using portals. How? And about Weatherlight, can it planeswalk anywhere on its own? In Time Streams, it seemed Urza was required but in Mercadian Masques, it seemed to planeshift on its own.

2. When did Urza lock the Dominarian nexus and what does that mean? Croag and Davvol and even an army of Phyrexians  teleported to Dominaria on several occasions. If so, why can't they transport their entire army? Why do they need Rath?

If answers to some of my questions aren't given anywhere, could you please give me your own?

By the time of Mercadian Masques, the crew of the Weatherlight had aquired the pieces of the Legacy needed to enable planeshifting capability. Similarly, the Phyrexians had ambulators that let them travel between worlds. How they were shut out of Dominaria despite having ambulators is a little... vague. It's possible they just didn't have ambulator technology at that point.

They needed Rath because, well, it's just strategically smart. They can instantly transport an entire massive army to the plane with no warning, while also dramatically changing the terrain to suit their needs. It might not have been essential (although the losses Phyrexia suffered at Koilos suggest otherwise) but it makes a lot of strategic sense.

Also, it's cool.

As far as the Nexus is concerned, that wasn't actually deliberate. Urza accidentally disrupted that entire region of the Blind Eternities with the sylix blast. It was locked purely by chance. Had the Nexus not been broken by Freyalise, Dominaria would have been safe from the Phyrexians. Whoops.

By the same token, it would have remained in an Ice Age for what amounts to eternity. So take that with a grain of salt. Also, I feel like it would have eventually broken down, since planes like Shandalar that were slowly drifting in/out of the Shard.

Phyrexia had sleeper agents operating on Dominaria for an untold big-ass amount of years, as preparation for the big invasion; they had also been staging smaller war campaigns on the planet whenever necessary, such as against the Capashens or the Bloodline-Keldons. I'm not sure if the Nexus had shut Phyrexia out; I'm inclined to think it did but I think Urza found Xantcha before he discovered Dominaria was sealed off and my mind placed her rescue on Dominaria itself, though on hindsight that's probably not actually true. In any case, the campaigns staged by the Evincars took place long after the nexus was shattered.

The capabilities of Phyrexia are about as inconsistent over the novels as those of the planeswalkers are, once you start paying attention to it. Such as Rath being strategically advantageous even though they obviously managed to ambulate millions of troops to that planet in what was a very short time span as per the time gap between Nemesis and Invasion.
