Adding Traps to the initiative order

Some traps (e.g. Phantasmal Maw) roll initiative after combat has already begun.

Does anyone know how to place the trap in the initiative order after Monsters and Characters have changed the initiative order due to delayed turns and readied actions?

Keep careful track of same is the RAW answer. When you delay you delay till after someone's turn, their init is your new init. So if you delay till after your friend who goes on 18, your init is now 18 and after your friend. Readying is similar, you're new init point is identical to whoevers turn it was when your action triggered, only instead of after you're before them. So same deal, you ready to stop a charge. Enemy charges you on 18 init, you're new init is 18 but before the monster.
