Smallpox is clearly very good in this format.  It's a beating against Zoo, stops Thepths and Scapeshift from developing their mana, and is just generally a very good card.  However, there really isn't a well-established deck playing Smallpox at the moment.   There are many ways to build around it: discard, land destruction, a The Rock-esque deck, and even a Loam deck with Raven's Crime and the like.  Personally, I think that a GB build looks really good: Tarmogoyf, Tombstalker, Thoughtseize, Pulse, etc.  The problem is, I'm not sure how to fill out the rest of the deck.

Ideas?  Decklists?  Anything?
You are so very right, It breaks any advantige depths gets with a big turn 2 play. No land no critters for you. There have been 2 recent decks at PTQ San Jauns that won the event. One at Millford the other in Cordoba, Argentina.

Sort of. I tested a lot against DD/Thopter and found it a pleasing matchup. You can't answer a resolved thopter foundry but by destroying all lands you're still in it. I had this deck locked down to a Chrome Mox and 1 land to make tokens each turn and it was a brutal game, but I won.

I then lost another game where he was in the same setup but I couldn't find a Tombstalker to apply pressure.

I don't feel compelled to test Zoo. That matchup reeks of 90%+

I'm currently playing a mono-black "Pox" deck with Bloodghasts, Abyssal Persecutors, a Beseech the Queen toolbox (to fetch singletons of cards like Rite of Consumption, Death Cloud, Tendrils and Unmake) and a lot of sac-effects obviously.

It's quite fun to play, but as always going mono black ends up being a bit weak. I'm currently considering adding some white for sideboard options, but red might also work well with the theme.
They see me trollin', they hatin'. Only 31.0% of the playerbase actually prefers Standard to the other formats, yet it is the format that gets the most attention. PATHETIC. I've made some more adjustments to my list and its getting better.

In my testing Bloodghast was the most unexciting card I could draw unless I had a smallpox in my opening hand. And its lack of impact on the board, and purely aggressive nature in a mid/range control type deck made him awful. I clung to him endlessly... until I figured out the bad and dropped him entirely.

Persecutor is fun and games, but there's so many other dynamic and less damning options in Extended so you don't have to use him.

Smalpox is good. And i agree with a sentiment above that monoblack leaves something to be desired.

I'm workign on a new deck with smallpox in myself, but it's going a very different direction.

I think it best fits in a classic deathcloud type of a build. I do not think a classic Rock type of a deck should run Smallpox. You need a Loam engine in place to consistently play Smallpox and get measurable advantages. The loam engine synergizes with Deathcloud's archetype quite well imo.

This seems like the best plan to me. Maybe a small red and white splash with most of the deck black?

White: Ghostly Prison, Path (Though this seems a little unintuitive in a LD deck, testing anyone?)
Red: Bolt, Boom/Bust
Black: Bob, Smallpox, Thoughtseize, Gatekeeper, Smother, Hippy etc...
Land: 5 Fetches, 3 Flagstones, maybe 5 Rav duals, rest basic
